Ectopia Lentis in Hypermobile EDS: Signs, Symptoms, and Treatment Options

Ectopia lentis, a condition where the eye’s lens shifts from its normal position, can be a challenging complication for people with hypermobile Ehlers-Danlos syndrome (hEDS). While hEDS primarily affects joints and connective tissues, its impact on the eyes is often overlooked. Recognizing the signs and symptoms of ectopia lentis early can help manage vision problems and improve quality of life.


Ectopia lentis occurs when the lens inside the eye moves out of its normal position. The lens is held in place by tiny fibers called zonules, which can weaken or break in connective tissue disorders like hEDS. Since hEDS affects collagen and other connective tissues, the zonules may become fragile, increasing the risk of lens displacement.


Although ectopia lentis is more commonly associated with other types of EDS, such as classical or vascular types, it can also occur in hypermobile EDS. The degree of lens displacement varies from mild shifts to complete dislocation, which can cause blurred vision, double vision, or even more serious eye complications.


People with hypermobile EDS should be aware of the following signs that might indicate ectopia lentis:


  • Blurred or distorted vision: A shifted lens changes how light focuses on the retina, causing vision to become unclear.

  • Double vision: Misalignment of the lens can create overlapping images.

  • Eye discomfort or strain: The eye may feel tired or sore from trying to compensate for the lens displacement.

  • Changes in eye appearance: Sometimes the lens shift can cause visible changes, such as a pupil that looks irregular or off-center.

  • Sensitivity to light: Increased glare or halos around lights can occur.

  • Frequent changes in prescription glasses: Rapid shifts in vision may require frequent updates to corrective lenses.


If you notice any of these symptoms, it is important to consult an eye specialist experienced with connective tissue disorders.


Diagnosis starts with a thorough eye exam. An ophthalmologist will use specialized tools to examine the lens position and check for any displacement. Tests may include:


  • Slit-lamp examination: A microscope with a bright light to view the lens and surrounding structures.

  • Dilated eye exam: Eye drops enlarge the pupil to get a better view of the lens.

  • Ultrasound biomicroscopy: Imaging to assess the zonules and lens position.

  • Visual acuity tests: To measure how well you see at different distances.


Because ectopia lentis can be subtle in early stages, regular eye check-ups are essential for people with hypermobile EDS.


Treatment depends on the severity of lens displacement and the symptoms experienced. Options include:


Corrective Lenses


For mild lens shifts, glasses or contact lenses can help focus light properly on the retina. Special lenses may be prescribed to reduce double vision or improve clarity.


Eye Patching or Prism Lenses


In cases of double vision, patching one eye temporarily or using prism lenses can help align images and reduce discomfort.


Surgery


If the lens is significantly dislocated and causes severe vision problems, surgery may be necessary. Surgical options include:


  • Lens repositioning: Moving the lens back into place.

  • Lens removal and replacement: Removing the displaced lens and implanting an artificial one.


Surgery carries risks, especially for people with connective tissue disorders, so it requires careful planning and an experienced surgeon.


Managing Associated Eye Conditions


Ectopia lentis can increase the risk of other eye problems such as glaucoma or retinal detachment. Regular monitoring and early treatment of these conditions are crucial.


Living with Ectopia Lentis and Hypermobile EDS


Managing ectopia lentis involves more than just treating the eye. People with hypermobile EDS should:


  • Schedule regular eye exams to catch changes early.

  • Protect eyes from injury, as fragile connective tissues increase risk.

  • Communicate openly with healthcare providers about vision changes.
